Daily Services include: Participation in day habilitation program model, running from 8:30 AM until 3:30 PM, which includes large and small group activity planned by our Activities Director and one on one activity with individual staff member. Assistance with activities of dailing living, including toileting (toileting schedule and reminders), eating, dressing, and grooming. Nursing care supervised by full time Registered Nurse, including medication administration, treatments (blood pressure, weights, glucose testing if needed), and daily monitoring of wellness. Care planning and caregiving instruction to families which utilizes input of families, doctors, activities director, registered nurse, OT, PT and Speech therapists (if utilized), and other program professionals. Continental style breakfast and a hot or cold nutritious lunch (accomodations for dietary restrictions) and one morning and one afternoon snack. Caregiver newsletter every other month and a lending library of caregiving resources. Free Family Support Group meets the last Tuesday of each month. Special services (at no charge*): Occasional outings to parks, shops, or musical presentations (two- clients- to-one- staff/volunteer ratio to be maintained on these trips) Multi-generational activities organized with school children Special "guest" musicians/ and storytellers to entertain on special occasions. Occasional "pet" therapy (allergic clients to be involved in alternate activity at that time) *Occasionally, we may take a trip out to a restaurant that would require the client to bring along his/her money for a meal. Senior discount will be arranged in advance. Menu of Services Daily fee: includes the hours from 8:30 AM to 3:30 PM (Monday through Friday, patients accepted for a minimum of two days per week-we do not offer a one day option) Additional services available

Referrals to Home Health providers available upon request for night care help in your home We are a private pay facility, but we have a contract with the US Veteran's Administration. We have access to grants and can offer special financing arrangements to make day care an affordable option for families. We do not accept Medicaid. |
